KAJIAN EPIDEMIOLOGI KLB DIFTERI DI PUSKESMAS GATTARENG KABUPATEN BULUKUMBA

Abstract

ABSTRACT

According to WHO data, global diphtheria cases experienced an increase in 2019 by 22,986, then decreased in 2020 to 10,107 cases. In 2020 Southeast Asia the number of diphtheria cases was 4,002 which made Southeast Asia the second largest after Africa with a total of 5,387 cases of diphtheria. Diphtheria cases in 2020 have spread to almost all regions in Indonesia. The number of diphtheria cases in 2020 was 259 cases (CFR 5.02%). Diphtheria cases in Prov. South Sulawesi in 2019 as many as 10 people (CFR 0.0%). And in 2020 there was 1 case of diphtheria (CFR 0.0%). In 2021, 1 case of diphtheria was found in Bulukumba Regency. This type of research is descriptive by looking at the history of immunization coverage that has been obtained by DPT/HB-Hib, Additional DPT/HB-Hib Immunization and coverage of Dt Class 1 and Td Class 2 and class 5. Determination of cases of Diphtheria Outbreaks in Kab. Bulukumba in 2021 is positive for non-toxigenic Corynebacterium diphtheriae belvanti (+). And there are 2 cases of clinically compatible cases from 2021 to February 2022. Coverage of DPT/HB-Hib immunization, DPT/HB/Hib Advanced Immunization and BIAS Dt Td in Gattareng Village and Benteng Malewang Village from 2014 to 2017 still below 95%. DPT/Hb-Hib immunization coverage, DPT/Hb-Hib Advanced Immunization and BIAS Dt Td below 95% are risk factors for diphtheria events, populations at high risk of contracting diphtheria, namely groups of children under 10 years old who live in areas with immunization coverage of less than 95% and adults are also at risk because they have a complete history of previous immunizations.

Menurut data WHO, kasus difteri secara global mengalami peningkatan tahun 2019 sebanyak 22.986, kemudian mengalami penurunan pada tahun 2020 menjadi 10.107 kasus. Pada tahun 2020 Asia Tenggara jumlah kasus difteri sebanyak 4.002 yang menjadikan Asia Tenggara menempati posisi kedua terbanyak setelah Afrika dengan jumlah kasus difteri sebanyak 5.387 kasus. Kasus Difteri pada tahun 2020 menyebar di hampir semua wilayah di Indonesia. Jumlah kasus difteri pada tahun 2020 sebanyak 259 kasus (CFR 5,02%). Kasus difteri di Prov.  Sulawesi Selatan Tahun 2019 sebanyak 10 orang (CFR 0,0%). Dan pada Tahun 2020 ada 1 kasus difteri (CFR 0,0%). Pada Tahun 2021, ditemukan 1 kasus difteri di Kabupaten Bulukumba. Jenis penelitian ini adalah deskriptif dengan melihat riwayat cakupan imunisasi yang pernah didapatkan DPT/HB-Hib, Imunisasi Tambahan DPT/HB-Hib dan cakupan Dt Kelas 1 dan Td Kelas 2 dan kelas 5. Penetapan kasus KLB Difteri di Kab. Bulukumba pada tahun 2021 yaitu positif corynebacterium diphtheriae belvanti (+) non toksigenik. Dan terdapat 2 kasus kasus kompatibel klinis tahun 2021 s/d feb tahun 2022. Cakupan imunisasi DPT/HB-Hib, Imunisai Lanjutan DPT/HB/Hib  dan BIAS Dt Td di Desa Gattareng dan Desa Benteng Malewang dari tahun 2014 s/d tahun 2017 masih dibawah 95%. Cakupan imunisasi DPT/Hb-Hib, Imunisasi Lanjutan DPT/Hb-Hib dan BIAS Dt Td dibawah 95% menjadi faktor resiko kejadian Difteri, populasi resiko tinggi tertular Difteri yaitu kelompok anak-anak usia dibawah dari 10 tahun yang tinggal diwilayah cakupan imunisasinya kurang dari 95% dan usia dewasa juga beresiko karena memiliki riwayat imunisasi sebelumnya yang lengkap.
